• Please provide your full legal name, date of birth, and gender as they appear on your passport. Suffixes/Titles are NOT NECESSARY to include even if listed on your passport.

• The legal name you provide is used to book flights and will be on your boarding passes. This name MUST MATCH YOUR PASSPORT or you may be denied boarding your flights.
